All incidents requiring first aid must be reported immediately by calling Campus Security at Local 7770 or 1-855-282-7770.
The affected/injured person, their representative or a witness to the incident should complete the Incident Report Form. Please complete this form providing factual and accurate information only, and then forward it to the Occupational Health and Safety Office within 24 hours of the incident.
