Initial 912 hour probationary evaluation
Prior to the completion of six months (912 hours) of the probationary period, the exempt supervisor will complete the 912 Hour (Six Month) Evaluation Form and arrange a meeting to review this document with the employee.

This meeting should take place prior to the completion of six months (912 hours). The exempt administrator should request that the employee come prepared to discuss and reflect on their progress during the first five months.
Once reviewed, the signed 912 Hour (Six Month) Evaluation Form is to be returned to Human Resources.
