Absence Form (Fillable), Absence Form (Non-Fillable) - for reporting absences to your supervisor

Health Spending Account Claim Form (Excel) (PDF)- for submitting claims to your insurance for reimbursement.

Development & Training Claim From (PDF) - for the reimbursement of development and training costs incurred.

Tuition Waiver Application (pdf) - tuition waiver application for employees, their spouses, and dependants.

2009 - 2010 Vacation Request Form - to request vacation from April 1, 2009 to March 31, 2010 2010-2011 Vacation Request Form - to request Vacation from April 1, 2010 to March 31, 2011.

Employee Change of Address Form (Fillable pdf) Employee Change of Address Form (Non-Fillable pdf) if you have recently moved, please fill out and submit this form to Employee Services so that we may update our records.

Faculty Sabbatical Leave Application Form 2010/2011 - Please note October 31 is the annual submission deadline.  Applications for Sabbatical Leave are processed in the fall semester preceding academic year in which the leave falls.  If any questions or concerns arise please contact Nancy Scarrow (local 4065).

PD Application - is not available online. To pick up an application form please contact Laura Chomiak (local 5133). They will assist you with any questions you may have, will guide you on how to properly fill one out, and inform you of the necessary supporting documentation required.
