Application Deadlines:

Applications for winter and fall intake are accepted on a continual basis until seats fill.

Complete your application

  1. Fill out the UFV Application form online. See HCA under Vocational programs in the drop-down menu. 

*Please note that to be considered official, transcripts must be sent directly to UFV from the originating institution

Submit additional documents

Submit all remaining documents to the School of Health Studies, Chilliwack campus. You can send in person, by email, or fax.

  1. Additional HCA Application Form (Updated June 2019)
  2. HCA Program English Competency Self-Declaration Form (Updated June 2019)
  3. Standard First Aid with CPR-C or Emergency First Aid with CPR-C or CPR-HCP
  4. Work or personal reference letter
  5. A valid driver’s licence and access to personal transport is normally required for the practicum course and other practice experience components. If students are unable to drive, they will be unable to meet the Health Care Assistant program requirements.
