Disclosing is when someone shares that they've experienced sexualized violence with a person they trust, such as a friend, counselor, or staff member. Disclosure is about seeking support, not necessarily starting a formal process.
Reporting is when someone formally notifies an authority, like the university or the police, about an incident of sexualized violence. Reporting starts and official process that may involve an investigation or action.

What happens after I disclose?
- A Student Support Navigator will reach out to you within three business days.
- The Support Navigator will inform you about all the reporting options and processes that you can choose from.

- By using this form, you are not submitting a report, and therefore not starting a formal process under UFV’s policy on Sexualized Violence, however the Support Navigator will provide options for you to consider.

If you do not wish to connect to a Specialist or Support Navigator at this time and are ready to make a formal report under this policy, please use the Incident Reporting form.
If you require emergency assistance, please call 911. The Disclose sexualized violence form is not to be used in an emergency or during an event(s) presenting an immediate threat to the health or safety of yourself or any campus member.
Students living in student housing may also contact Campus Security in an emergency (to be connected with the Residence Manager on-call).
