Help keep UFV safe. Use the forms below to report a security issue, incident, near miss, or hazard. Your report allows us to respond and take appropriate action.

Report security concerns, injuries, threats, theft, suspicious activity, or a near miss that could have caused harm.

How to report an incident or near miss

If you need immediate help

  • In an emergency, always call 911 first (9-911 from a campus phone)

Tell the appropriate person

Notify your work supervisor or instructor about the incident as soon as possible after it happens.

Submit a Safety & Security incident form

Fill in and submit a Safety & Security incident report form within 24 hours. A copy of the form will be sent to your supervisor and EHS.

Types of incidents to report

  • A first-aid incident
  • An injury from a traffic accident
  • Illness from exposure to a substance on campus
  • A "near-miss" that could easily have caused harm
  • An accident or injury during a UFV field trip
  • A threat, assault, or other violence
  • Theft or vandalism
  • Suspicious activity
  • Sexualized violence (see below for reporting details)

How to report a hazard

Notify your work supervisor or instructor about the hazard and collaborate on a solution.

Submit a Safety & Security hazard report form

Use the Report a hazard form to notify Environmental Health & Safety. This helps us to be aware of unsafe conditions on campus and develop broader responses.

Types of hazards to report

  • An icy patch on a UFV walking path
  • A broken light switch or exposed wire
  • A wet floor
  • An area of campus that is poorly lit
  • A chemical that is not being stored safely
  • Something that might cause someone to trip and fall
