How to get a Campus Card

Winter semester card issuance starts January 2, 2024.
Register and be accepted in at least one course in the current semester.
Not sure what to study? Find your program.

Ensure all your forms have been submitted to Human Resources.
If you require door access, ask your supervisor/dept. head to authorize the Key Request Form‌.

Ensure UFV has your correct legal name on file

Your legal name in UFV's records must match the government-issued photo ID presented to the Campus Card office. If you have more than one legal last name, such as your birth and a married surname, you must select one name for UFV's records and present a valid government-issued photo ID with the chosen name displayed.

For assistance changing your student or employee record:

Students complete the Student Name Change Form and visit the Office of the Registrar.

Employees complete the Employee Name Change Form and visit the Human Resources office.

Make sure your address is up-to-date

Log-in to myUFV and update your address.
Once you've logged-in select > Personnal Information > Update Address and Phone.

Complete the Employee Change of Address Form.
Send the completed form to Human Resources to change your address on file.

Have your UFV ID number ready

Your nine-digit ID number is your unique identifier at UFV.

Students your ID number was assigned to you when you first applied to UFV.

Employees your ID number was assigned to you by Human Resources.

Provide valid government issued photo ID
