What is the Wellness Report?
- The Wellness Report (TWR) is a short voluntary, confidential survey designed to help us better understand UFV’s health and identify areas for improvement, through UFV's benefits provider Manulife.
- It is a comprehensive workplace wellness assessment tool that collects information from employees through an online survey. It measures physical, psychological, financial, and organizational health and wellness, as well as the connection between individual and organizational health.
Why should I participate?

The Wellness Report benefits both UFV and its employees:
- It offers employees a Personal Health Report to help them better understand their health needs, along with recommendations for leading a healthier lifestyle, based on the responses they provide.
- It provides an organizational assessment, looking at existing workplace wellness measures and their effectiveness, while also providing actionable insights and recommendations for improvement.
Will my answers be confidential?
- Your personal Wellness Report is linked to your survey URL and not your name and position at UFV.
- Manulife uses anonymized data to provide sector-wide recommendations to support other organizations similar to UFV.
When is it taking place?
- The survey runs from October 1 to October 31, 2025.
- You will receive your own Personal Health Report approximately two weeks after completing the survey.
How can I participate ?
- Follow the link that you were emailed and complete the 15 minute survey (starting October 1).
Collection Information
Manulife will administer the Wellness Report Survey through its survey provider, Deloitte. The survey is subject to Manulife’s Terms and Conditions and any information you provide will be subject to Deloitte’s Privacy Policy. The survey is confidential and anonymous—your individual answers will never be shared with UFV or Manulife.
As a result of your participation in the survey, UFV’s Human Resources department will be provided with an Organizational Health Report that will provide a comprehensive overview of UFV’s health and wellness, and you will be provided with your own personal Wellness Report. Your personal Wellness Report is linked to your survey URL and not your name and position at UFV. Manulife uses anonymized data to provide sector-wide recommendations to support other organizations similar to UFV.
