Your input is needed 

An important survey on the future of health care availability at UFV is being conducted as part of a faculty sabbatical project, and we need employees to weigh in before August 15, 2024. 

Focusing on accessibility of health care for UFV students and employees, this survey seeks to determine whether medical services on-campus are needed and would be utilized. It should take 10 minutes to complete and will inform a recommendation for how UFV could move forward to provide health care services for students and employees in the future. 

Your participation is voluntary and anonymous. All surveys submitted will be utilized in the data analysis. Please answer any/all of the questions that you are comfortable answering.
