Unpaid Leave of Absence (Short - less than one month)
Unpaid Leave of Absence (Long - longer than one month)
An employee may apply for general leave by filing a written application to the President (please refer to Article 25.5 in the collective agreement). For more information please contact Nancy Scarrow.

Request for Contract Form


Sick leave requests:

Sick Leave Form - Please complete for all absences.

Doctor Certificate - To be completed by a physician for all Sick Leave Absences greater than 4 consecutive work days. This is now the only format of medical note that is accepted.

For all absences greater than 5 working days, please contact Laurie Coreau or (604-557-4014, local 4014 internally), Health & Disability Coordinator.
